I understand that being a part of Dance Migration intensive school programs means I am committed to working with my group and my teachers with a positive and supportive attitude. I will respect the health and safety of my fellow classmates and will not attend class if I am experiencing any cold/flu like symptoms.

I do hereby release Dance Migration & Studio and all instructors and students in any capacity for any liability due to injuries, illness etc. that may obtain as a result of my attendance and participation in any and all dance classes at Dance Migration or any specific events associated with this class. I clearly understand that participation in these classes will involve dance and bodily exercise and movement with rigorous activity. I am fully aware of my personal medical condition and hereby certify that I am mentally and physically fit to participate in the classes for which I have registered and that I have medical coverage for personal means to cover the expenses related to any injury that I may receive as a result of my participation. Dance Migration is not responsible for any lost or stolen items.

MEDIA CONSENT *

I hereby authorize any images or video footage taken of myself in whole or in part, individually or in conjunction with other images and video footage, to be displayed on the Dance Migration website or other official channels, and to be used for media purposes including promotional presentation and marketing campaigns. Also any choreography or class videos taken I understand to give credit to Dance Migration and teachers for their work.
